Ummmm no, the closest in terms of family tree is his half-blood sister(your wife) and his blood relative uncles and aunts. You're only his brother in laws, not related by blood. You may be in his family tree, but certainly not the closest. Isn't the closest always blood relatives first and then in laws? Or am I wrong?
I get that maybe he doesn't want to tell the other family members (or maybe they just don't care anymore) about his conditions, so he's the one who becomes his guardian. But could you legally took the guardian position as the in laws while there's still the blood relatives?
